ABSTRACT

Introduction: Open bite can generate relevant psychosocial impact in the daily life of the patient, both from the aesthetic and functional point of view. Objective The aim of this study was to estimate the prevalence and factors associated with the anterior open bite in children in the first school year in a municipality in southern Brazil. Material and method A cross-sectional study was carried out with six-year-old schoolchildren in Palhoça/SC. Interviews were conducted with mothers to gather socioeconomic and non-nutritive sucking habits information. Children oral clinical examinations were carried out in schools. Multivariate analyses were performed using Poisson regression with a robust estimator. The study involved 655 children, 50.5% being female. Result The prevalence of anterior open bite was 14.1% (95% CI 11.5; 16.7). Girls exhibited a lower prevalence [PR= 0.96 (95% CI 0.94; 0.99) p= 0.024] and those children who used a pacifier exhibited a higher prevalence of anterior open bite [PR= 1.04 (95% CI 1.01; 1.07) p= 0.003]. Conclusion The prevalence was 14.1% and that it was associated with gender and the use of pacifier, in a statistically significant and independent way.


Introdução: A mordida aberta pode gerar relevante impacto psicossocial no cotidiano do paciente, tanto do ponto de vista estético quanto funcional. Objetivo Estimar a prevalência e os fatores associados à mordida aberta anterior em crianças do primeiro ano escolar de um município do sul do Brasil. Material e método Estudo transversal constituído por escolares de seis anos de idade de Palhoça/SC. Foram realizadas entrevistas com as mães para obtenção de informações socioeconômicas e de hábitos de sucção não nutritivos. Exames clínicos bucais das crianças foram realizados nas escolas. Análises multivariadas foram realizadas por meio de regressão de Poisson com estimador robusto. Resultado O estudo envolveu 655 crianças sendo 50,5% do sexo feminino. A prevalência de mordida aberta anterior foi de 14,1% (IC 95% 11,5; 16,7). Meninas apresentaram prevalência menor [RP= 0,96 (IC 95% 0,94; 0,99) p= 0,024] e aqueles que utilizaram chupeta apresentaram uma maior prevalência [RP= 1,04 (IC 95% 1,01; 1,07) p= 0,003]. Conclusão A prevalência foi de 14,1% associada de forma estatisticamente significativa e independente com sexo e utilização de chupeta.

ABSTRACT

O objetivo deste estudo exploratório foi conhecer a perspectiva dos cirurgiões-dentistas da rede de atenção básica à saúde de Florianópolis acerca da necessidade, viabilidade e interesse quanto à ampliação dos serviços de ortodontia preventiva e à implementação dos de ortodontia interceptativa nas Unidades Básicas de Saúde do município. Empregou-se um questionário estruturado para coletar as opiniões de todos os cirurgiões-dentistas da atenção básica que realizam atendimento clínico na rede. Os resultados demonstraram que: a maioria deles é favorável desde que haja uma estruturação adequada para atender à necessidade da população; que os profissionais se consideram despreparados para realizar os procedimentos necessários, de modo que haveria a necessidade de capacitações; que não haveria a necessidade de se implantar outros procedimentos de ortodontia na atenção básica além dos analisados; e que as principais dificuldades que seriam enfrentadas estariam relacionadas ao excesso de demanda e à falta de recursos humanos. Apesar destes obstáculos, a maioria dos profissionais encara como positiva e viável a ampliação/introdução desse serviço, o que favoreceria a integralidade da atenção à saúde e fortaleceria a atenção básica, melhorando assim a saúde da população.


The scope of this exploratory study was to understand the viewpoint of the dental surgeons of the Primary Health Care (PHC) network in Florianópolis regarding the need, feasibility and interest in broadening preventive orthodontic services and in the implementation of interceptive orthodontics. A structured questionnaire was used to establish the viewpoint of all the primary care dental surgeons who perform clinical care in the network. The results indicate that: the majority are in favor provided that there is an appropriate structure to attend the needs of the population; the professionals consider themselves unqualified to perform the requisite procedures, so training would be necessary; there would be no need to deploy other orthodontic procedures in primary care than those analyzed; and the main difficulties that might be encountered were related to excess demand and the lack of human resources. Despite these obstacles, the majority of the professionals consider it positive and feasible to introduce this service, which could increase the comprehensiveness of health care and bolster primary care, thereby improving the health of the population.

ABSTRACT

Apesar da alta prevalência de oclusopatias em adolescentes ser mundialmente relatada, poucos estudos investigaram a associação entre oclusopatias definidas por critérios clínicos e a auto-avaliação da aparência dentária e das gengivas em adolescentes. O objetivo deste estudo foi investigar a associação entre oclusopatias e auto-avaliação da aparência dentária e gengival em adolescentes brasileiros. Foram utilizados os dados de um estudo transversal realizado com adolescentes de 15 a 19 anos de idade (n = 16.126) de 250 cidades localizadas nas cinco macrorregiões do Brasil. O desfecho foi a insatisfação com a aparência dentária e gengival, sendo a principal variável exploratória as oclusopatias, medidas através do Índice de Estética Dental - DAI. As demais variáveis exploratórias, potenciais fatores de confusão e mediação, foram renda familiar per capita, atraso escolar, condição de estudo, sexo, idade, cor da pele, agravos bucais (cárie não tratada, perda dentária devido à cárie, cálculo, fluorose e dor nos dentes e gengivas) e a utilização dos serviços odontológicos. Realizaram-se análises de regressão de Poisson simples e multiva-riável. A prevalência de insatisfação com a aparência dos dentes e gengivas foi de 11,4 por cento (IC 95 por cento: 10,4-12,5). Todos os graus de oclusopatias foram associados à insatisfação com a aparência dos dentes e gengivas. A análise multivariável ajustada mostrou que a auto-avaliação negativa da aparência em adolescentes com oclusopatias graves ou muito graves foi 40 por cento e 80 por cento maior quando comparados àqueles com oclusão normal, respectivamente. Os resultados contribuem para a inclusão do critério de auto-avaliação da aparência dental durante as decisões de tratamento ortodôntico, principalmente no âmbito do Sistema Único de Saúde.


In spite of the high prevalence of malocclusion in adolescents reported worldwide, there are few studies that have investigated the association between normative malocclusion and self-rated dental and gingival appearance among adolescents. The aim of this study was to identify the association between normative malocclusion and dissatisfaction with dental and gingival appearance among Brazilian adolescents. A cross-sectional study was carried out with adolescents aged 15 to 19 years (n= 16,126) living in 250 towns of all five Brazilian regions. Dissatisfaction with dental and gingival appearance was the outcome. The main explanatory variable was malocclusion assessed by using the Dental Aesthetic Index - DAI. The other explanatory variables included were per capita family income, schooling delay, study conditions, sex, age, skin color, dental outcomes (untreated dental caries, missing teeth due dental caries, dental calculus, fluorosis, and dental pain) and use of dental services. Simple and multivariable Poisson regression analyses were performed. Dissatisfaction with dental appearance reached 11.4 percent (95 percentCI: 10.4-12.5) of the entire sample. All levels of malocclusion were associated with dissatisfaction with dental appearance. Adjusted multivariable analysis showed that dissatisfaction with dental appearance among individuals affected by severe or very severe malocclusion was respectively 40 percent and 80 percent higher than among those with normal occlusion. Malocclusion was associated with dissatisfaction with dental and gingival appearance. The results contribute to include self-rated dental appearance criteria in orthodontic treatment decision, mainly within the National Health System - SUS.

ABSTRACT

El presente trabajo contiene las diferentes fases del proceso de investigación realizado en el periodo de Enero a Diciembre 2009 en las Unidades de Salud de Guayapa Abajo, Unidad de Salud de Guaymango, Unidad de Salud Colonia Ista y Unidad de Salud San José el Naranjo del departamento de Ahuachapán, donde se realizó un diagnóstico de la condición de la salud bucal de la población que asiste a dichas unidades de salud. Este trabajo consta de un objetivo general y objetivos específicos en los cuales se planteó realizar un diagnóstico de la realidad estomatológica de la población en estudio. Por otro lado se desarrollan las variables socioeconómicas de la población que asiste a las unidades de salud, el identificar el motivo de consulta del paciente, detectar la existencia de lesiones de cara, cuello y osteomucofibrosas, determinar los índices dentoperiodontales, establecer las oclusopatías en la población, comprobar el problema estomatológico que más afecta por medio de una cédula de entrevista y una guía de observación que fueron dirigidas a los usuarios que asistieron a la consulta odontológica. En ese trabajo se presentan los resultados obtenidos de la condición de salud bucal de la población en estudio ya que por cada variable existe un análisis sistemático; adicionalmente se realiza un consolidado, discusión y conclusión de los resultados obtenidos de dicha investigación así también se realizó una serie de recomendaciones que se consideran necesarias.


This work contains the different phases of the research process carried out in the period from January to December 2009 in the Health Units of Guayapa Abajo, the Health Unit of Guaymango, the Colonia Ista Health Unit and the San José el Naranjo Health Unit of the department. from Ahuachapán, where a diagnosis of the oral health condition of the population that attends these health units was made. This work consists of a general objective and specific objectives in which it was proposed to make a diagnosis of the stomatological reality of the study population. On the other hand, the socioeconomic variables of the population that attend health units are developed, identifying the reason for the patient's consultation, detecting the existence of face, neck and osteomucosofibrous lesions, determining dentoperiodontal indices, establishing occlusopathies in the population, check the stomatological problem that affects the most by means of an interview card and an observation guide that were directed to the users who attended the dental consultation. In this work, the results obtained from the oral health condition of the population under study are presented, since for each variable there is a systematic analysis; additionally, a consolidation, discussion and conclusion of the results obtained from said research is carried out, as well as a series of recommendations that are considered necessary.

ABSTRACT

Esta investigación fue realizada con el objetivo de diagnosticar la condición bucal de la población que asiste a consulta odontológica a las Unidades de Salud de Santa Rosa de Lima, San Alejo y Agua Fría en conjunto, en el departamento de La Unión. El estudio es de tipo descriptivo detallando la población por rangos de edad, aspectos socioeconómicos, así como la aplicación de diversos índices epidemiológicos; y se llevó a cabo en el año 2009 en los meses de julio a octubre. La población de estudio fueron los pacientes que acudieron a la consulta odontológica en la Unidad de Salud Santa Rosa de Lima con una muestra aleatoria de 174 pacientes y de la Unidad de Salud de San Alejo y Casa de Salud de Agua Fría con 88 pacientes como muestra aleatoria. Se indagaron datos generales del paciente a través de una cédula de la entrevista, y se evaluó la condición bucal por medio de una guía de observación. De los resultados obtenidos los principales motivos de consulta fueron: problemas dentales con 49% y 63%, y problemas periodontales con 49% y 37%; la exostosis con 70% y 20% , y eritroplasia con 20%; el índice de placa bacteriana Loe y Sillness 47% con buen estado y 51% condición leve, para el índice gingival 58% y 48% con buen estado, en cuanto al índice periodontal comunitario presento cálculo supra o subgingival en el 50% y 70%, para el índice de estética dental DAI y el índice de maloclusión en niños hay predominio de la oclusión normal y maloclusión mínima y el índice CPO/D-ceo/d resulto alto. Se concluyó que los problemas dentales son el principal problema de las poblaciones de estudio.


This research was carried out with the objective of diagnosing the oral condition of the population that attends dental consultations at the Health Units of Santa Rosa de Lima, San Alejo and Agua Fría as a whole, in the department of La Unión. The study is descriptive, detailing the population by age ranges, socioeconomic aspects, as well as the application of various epidemiological indices; and it was carried out in 2009 from July to October. ABSTRACT

El presente trabajo es una investigación de campo de tipo descriptivo, cuyo objetivo principal fue, analizar la condición de salud bucal de la población de las unidades de salud de San Rafael Obrajuelo, San Pedro Masahuat, ambas de La Paz; Periférica de Sensuntepeque, Cabañas y San Ramón, de la región paracentral del Ministerio de Salud Publica y Asistencia Social; en el año 2009.Se aplicaron dos tipos de instrumentos para la recolección de los datos, la cédula de entrevista y una guía de observación. Los resultados permiten concluir que, la mayor parte de la población en el estudio corresponde al sexo femenino, oscilando el mayor porcentaje en el grupo etareo de 5 a 11 años; más del 50% de la población no tiene ingreso económico y si tienen es inferior al salario mínimo. Entre el 71% y el 86% de la población sabe leer y escribir y de ese el nivel básico de educación es el que la mayoría ha alcanzado. El motivo de consulta más encontrado fueron los problemas dentales, entre un 63% a 67% siendo la caries dental el origen de estos. El índice de placa bacteriana en tres de las Unidades de Salud prevalece entre un 43% y 63% la condición leve y en una, la condición moderada con un 47%. En cuanto al índice gingival se presentó inflamación leve en los cuatro establecimientos con un promedio de 55%.En el examen clínico de cara, cuello y mucosas, la eritroplasia es la lesión que prevaleció con un promedio de 35% sobre todo en las estructuras intraorales.En las cuatro Unidades de Salud, el resultado de CPO/D y ceo/d es muy alto, sus valores oscilan entre el 9.24% y el 14.66%. En cuanto al índice de estética y oclusión tres de las Unidades de Salud presentan oclusión normal y maloclusión mínima, compartida con maloclusión muy severa o discapacitante. En cuanto a la población infantil también prevalece la maloclusión mínima y solo un establecimiento presenta maloclusión definida y severa.


The present work is a descriptive field research, whose main objective was to analyze the oral health condition of the population of the health units of San Rafael Obrajuelo, San Pedro Masahuat, both of La Paz; Peripheral of Sensuntepeque, Cabañas and San Ramón, of the paracentral region of the Ministry of Public Health and Social Assistance; in 2009, two types of instruments were applied for data collection, the interview card and an observation guide. El Salvador es un país subdesarrollado con condiciones de vida inadecuadas y un sistema de salud bucal deficiente, tal problemática se caracteriza por una alta necesidad de atención odontológica, por lo cual se realizo una investigación descriptiva, cuyo objetivo general es realizar el diagnóstico de salud bucal como paciente colectividad, de la población que asiste a la Unidad de Salud del Municipio de Metapán y Clínica Odontológica de la Facultad Multidisciplinaria de Occidente, de la Universidad de El Salvador departamento de Santa Ana, en el año 2009; tomando en cuenta también, otros objetivos específicos para dicha investigación tales como; determinar las variables socioeconómicas, los índices dentoperiodontales, las oclusopatías, detectar la existencia de lesiones osteomucofibrosas e identificar el motivo de consulta de la colectividad, para la recolección de datos se utilizo; cédula de entrevista y guía de observación. El análisis estadístico se realizo utilizando los programas EPIINFO v.3.5.1, Microsoft Access y Excel 2007; haciendo una revisión y adaptación de datos para el análisis, cruzando las diferentes variables, encontrando como resultados; que el principal motivo de consulta son los problemas dentales, las lesiones que están presentes son exostosis y eritroplasia; el Índice de placa bacteriana se presento en condición leve y buen estado; el Índice gingival en inflamación leve y buen estado; el Índice periodontal con hemorragia al sondaje suave y sano, el Índice de estética dental en oclusión normal y maloclusión mínima; el Índice de maloclusión en niños de 3 a 12 años con maloclusión mínima, definida y severa y el Índice CPO/D-ceo/d poblacional alto, en conclusión todo lo anterior refleja una población con condición bucal regular.

Esta investigación fue realizada con el objetivo de diagnosticar la condición de salud bucal de la población que asistió a consulta odontológica a las clínicas de la facultad de odontología de la Universidad de El Salvador en el año 2009; El estudio se basa en una investigación de tipo descriptiva donde se clasifico la población por rangos de edad y sexo; así mismo fue necesario mencionar aspectos socioeconómicos, donde se incluyen los aspectos relacionados con el nivel de estudio, ingresos económicos y lugar de residencia actual y de los primeros años de vida; de igual manera se realizó la aplicación de diversos índices epidemiológicos, con la finalidad de descubrir los diferentes tipos de problemas bucales por los que la población consulta. La población del estudio se obtuvo de los pacientes que acudieron a la facultad de odontología de la Universidad de El Salvador durante los meses de febrero y marzo del año 2009 donde el total fue de 412 personas, del cual se obtuvo una muestra de 86 pacientes para la investigación. Se obtuvieron datos generales del paciente por medio de una cédula de la entrevista, y se evaluó la condición bucal por medio de una guía de observación. Los resultados obtenidos muestran que el principal motivo de consulta es el dolor dental, seguido de la presencia de caries, por rehabilitación dental, por prevención, por malposición dental y por problemas periodontales; las principales lesiones encontradas fueron las exostosis, el índice de placa bacteriana LOE y SILNESS fue clasificado en buen estado, en cuanto al índice gingival LOE Y SILNESS (IG),se obtuvo que el mayor porcentaje de la población posee una condición gingival en buen estado; el índice periodontal comunitario fue de buen estado; para el índice de estética dental DAI se obtuvo que predominó la oclusión normal y maloclusión mínima y en niños menores de 12 años existe una maloclusión definida y severa; con respecto al índice CPO/D-ceo/d se obtuvo un índice alto; Se concluyó que los problemas de origen dental son el principal problema que afecta a la población.

El presente estudio busca realizar el diagnóstico de salud bucal como paciente colectividad de la población que asiste a las clínicas odontológicas y unidad de salud de: Hospital Militar Central, Cruz Roja Salvadoreña, Clínica Asistencial Corazón de María y Unidad de Salud de Panchimalco; así como determinar las variables socioeconómicas de la población, identificar el motivo de consulta del paciente, detectar la existencia de problemas en cara, cuello y osteomucofibrosas, determinar los índices dentoperiodontales; así como también las oclusopatías de la población con todo ello se pudo establecer el problema principal estomatológico de la colectividad. Esta investigación es de tipo descriptiva, la cual fue realizada en el año 2009, utilizándose los instrumentos de cédula de entrevista y guía de observación como método de recolección de datos en el período de septiembre a noviembre de dicho año. De las clínicas antes mencionadas se toma una muestra de 333 pacientes entre hombre y mujeres cuyas edades oscilan de 0 a 61 a más, dividiéndose éstas por diferentes rangos. Teniendo como resultados principales, que el género femenino fue el más predominante. Y el rango de edad donde se ubica la mayoría de la población es de 20 a 40 años. El motivo de consulta principal fueron los problemas dentales. Dando como conclusión que el problema principal encontrado en la población estudiada es el de caries dental el cual representa el 91.21%, mostrando así un nivel alto de índice CPOD/CEOD en la población.


This study seeks to make the diagnosis of oral health as a patient group of the population that treated the dental clinics and health unit of: Central Military Hospital, Salvadoran Red Cross, Corazón de María Assistance Clinic and Panchimalco Health Unit; as well as determining the socioeconomic variables of the population, identifying the reason for the patient's consultation, detecting the existence of problems in the face, neck and osteomucofibroses, determining the dentoperiodontal indices; as well as the occlusopathies of the population with all this it was possible to establish the main stomatological problem of the community. This research is descriptive, which was carried out in 2009, using the interview card and observation guide instruments as a data collection method in the period from September to November of said year. From the aforementioned clinics, a sample of 333 patients is taken between men and women whose ages range from 0 to 61 and over, dividing these by different ranges. Taking as main results, that the female gender was the most predominant. And the age range where the majority of the population is located is between 20 and 40 years old. The main reason for consultation was dental problems. Concluding that the main problem found in the studied population is dental caries, which represents 91.21%, thus showing a high level of the CPOD / CEOD index in the population.
